Community Rehabilitation

“Rehabilitation is a process which enables people to reach and maintain their optimal physical, sensory, intellectual, psychological and social performance. Rehabilitation gives people the tools they need to reach their independence…” (WHO, 2015).

The role of Community Rehabilitation Programs is to optimise a person’s functional ability and independence.

Community Rehabilitation offers short term goal directed rehabilitation of approximately 6-8 weeks, aimed at improving client’s function and independence following a change in function due to recent acute medical / health event.

This is achieved by developing a care plan based on goals that are negotiated between the client and their team (see below for team details).

Therapy can either be delivered in a group or individual setting. Clients can be seen in their own home or in one of our centres, dependent on their needs and goals.
